Your First Visit

We appreciate the opportunity to provide your dermatology care, and we look forward to meeting you. You can download our new patient forms here. The forms are fillable, so you can complete them on your computer and bring them with you, or print and fill them out by hand. If you prefer, please arrive 15 minutes early for your first appointment to complete them in the office.

Our office is located at 9675 Brighton Way, Suite 410, Beverly Hills, CA 90210. There are parking lots nearby that offer two hours of free parking.

Insurance

We are not participating providers for any insurance plans.

Payment in full is due at the time of each visit. We will provide you with a superbill that you can submit to your insurance company for reimbursement.

Cancellation Policy

Please cancel or reschedule at least one business day before your appointment. Late cancellations and missed appointments are subject to a $125 fee.

Product Return Policy

If you are dissatisfied with a product purchased from our office, we will gladly exchange the product or apply a credit to your account, provided it is within two weeks from the date of original purchase.

Please note: Products that are less than ¾ full will not be exchanged or credited for any reason. Prescription products, such as Latisse and Altreno, are not returnable for any reason.
